St Giles Surgery building hosts two separate practices operating a rota for registration which alternates 2/3 weekly cycles. The surgery operates an open list.
We are pleased to welcome new patients, We welcome all people regardless of their ethnic origin, gender,age,ability, class, religion or sexual orientation.
If you live outside of the catchment area, unfortunately you will have to find another GP surgery in your local area. Please check our map here.
All new patients are invited for a short health check with the nurse or health care assistant when they ask to register. We are unable to register you fully until this has been done. Please contact the surgery to book an appointment for new patient health check.
It is a valuable way of meeting you and establishing continuity of care; your previous medical records may not reach us for several weeks.
All new patients are requested to complete the online registration form and new patient questionnaire or come to into the surgery and complete the hard copy registration form and questionnaire.
When completing the form online, it is important that you put your NHS number on the form if you have been registered previously with another practice. We would also kindly request that you to bring in photo ID and proof of address during your first visit at the practice.
All patients have a named GP but are free to choose to see any of the doctors.
We accept all patients on our list regardless of their race, culture, religion or sexuality. To register please complete a registration form and health and ethnicity questionnaire which is available at the desk. You may be asked to provide proof of identity and of address but this may not always be necessary.
Return your completed registration form to reception along with your photo id (passport or valid UK drivers licence) and a proof of address ( utility bills or any other document your address within the last 3 months).
A previous NHS card would be very helpful.
We do reserve the right to remove any patient(s) from our list in the event of abusive, racist or intimidating behaviour at the surgery.
Once you are accepted onto the list we will invite you for a health check with one of the nurses in order to complete basic medical history and carry out any appropriate screening tests.
All new patients are welcome.

As part of the commitment to more personalised care for patients, NHS Employers and the General Practitioners Committee of the British Medical Association have agreed that all patients will have a named accountable GP
The 'named' GP will: -
- Take lead responsibility for ensuring that all appropriate services required under the contract with the practice are delivered to you
- Where required, based on the professional judgement of the 'named' GP, work with relevant associated health and social care professionals to deliver a multidisciplinary care package that meets your needs
- Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians in the practice
- Ensure that patients over 75 years of age have access to a health check if requested, which is already a requirement of the GP contract regulations.
St Giles Surgery will ensure that there is a named accountable GP assigned to each patient.
New patients will be allocated a GP at the time of registration.
Your named accountable GP will be the same as your USUAL GP however this does not affect your ability to see any GP of your choice as you currently do
