New Patient Registration
Our practice currently registers patients who live within our catchment area, which includes all of Westminster, Kensington and Chelsea, Wandsworth, Lambeth, and Hammersmith and Fulham. We also accept out-of-area registrations
To register at our practice, please download and complete the GMS1 form and relevant patient questionnaire available below and email it to us at drvmuirbelgravemc@nhs.net along with a copy of your ID (passport etc) and proof of address (utility bill etc). Alternatively, please bring proof of your address and either your NHS number/card or your passport to the surgery and the receptionist will provide you with a registration form to fill out.
For children under 10 years old, please also provide a copy of their immunisation records.
If you are on any medication or contraceptives, please bring them with you to your first appointment, as well as any other relevant information such as immunisation records, smear or mammogram results, to allow us to update your medical records.
Please make annual contact with the surgery to confirm continued registration with us.
Change in Circumstance
We will keep you informed of changes within the practice that are likely to affect you. Please let us know if you change your name, address, telephone number (including ex–directory numbers) etc so that our records for you and other members of your family are up to date. Failure to do so may result in you being taken off our practice list by the Health Authority.
Named GP for all Patients
From April 2015 all GP practices are required to provide all their patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.
Dr Muir is the named GP for all patients registered at the practice. She has overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the practice as you have always done.
