New Patient Registration

We accept patients from Lichfield and Burntwood.  To register, you will need to complete the necessary paperwork which is available at reception.  We ask for two forms of identification, one should be photo ID (such as passport or driving licence)  and the other to confirm where you live (such as a utility bill or bank statement).

On registration you will be invited for a new patient health check with one of our practice nurses.  It is important to attend as it helps to plan the care you might need.  You will be required to being a urine sample with you when you attend this appointment.

Which Doctor to See

Upon registration you will be given a “named GP” but you have the flexibility of consulting with any of the other doctors at the practice. However, we recommend you see the same doctor for any on-going medical matters to ensure continuity of care.
