Noticeboard

Dr Syme and Dr Watson are delighted to welcome Dr Tara Mill on board as partner. Some of you will have already met Dr Mill who  has been working at the practice since February, initially covering Dr Watson's maternity leave. In anticipation of his retiral in September 2016, Dr Syme will no longer be doing regular clinics. He will cover leave absences for Dr Mill and Dr Watson until he retires, so you may see him from time to time.

New Patient Registrationregister_now

If you wish to register, you will be asked to fill in a registration form and ‘new patient questionnaire’ and  attend the surgery to have a ‘new patient medical’ with the practice nurse.  When making your initial appointment, please ask for a double appointment and please remember to bring along a urine sample.

Patients will be offered an appointment with the doctor of their choice, at a time that suits the patient.  If their preferred doctor is not available, then an appointment will be offered with an alternative doctor.

Patients will be accepted for registration provided they live in the practice area.  The area covers Tyndrum, Crianlarich, Killin, Ardeonaig and Lochearnhead.  It also includes Glenfalloch to Inverarnan, Glen Lochay and Lochtayside (to the Lawers Burn).

Medical treatment is available from the date of registration. Please contact reception for further information.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

You will require the appropriate ID to register with the practice

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Online Medical Questionnaire For New Patients

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
