New Patient Registration
Our List is Open.
Anyone in England can register with a GP Surgery. You do not need proof of address or immigration status, ID or an NHS number.

Please note: completing the registration does not automatically mean you are registered at Cropredy Surgery.
Please ensure you have all your information to hand before starting the registration process as it needs to be completed in one go.
Cropredy Surgery Practice Boundary
We have extended our boundary and the link to the postcodes have not caught up yet.
Check on the map link at the end of this information for a rough guide to our included villages (please note, it is NOT 100% accurate due to the use of straight lines!)

If you would like to register at the surgery and are living within our Boundary area, ask reception for registration forms or complete the registration here
The forms require your name, address, NHS number (if you know it) and other information, such as, the name and address of your previous GP.
We will ask you for proof of identity, for example, a photocard driving licence or a passport and proof of your address such as a utility bill (gas, electricity, water or telephone bill but not a mobile telephone bill) or a council tax bill.
We also ask you to complete a health questionnaire and from that may ask you to come to the surgery for an appointment with the nurse. This will be to make sure that any tests or checks that you need are up to date.

Online Services
We offer our patients online appointment booking, repeat prescription requests and online records viewing through a system called Patient Access Read more.
You can ask to register for this service when you come to join the Practice.
Change of Personal Records
If you move house, change your name or telephone number please remember to let us know so that we can contact you, if necessary. You may telephone 01295 758372, email: Cropredy.Surgery@nhs.net or call in person to give us your new details.
