New Patient Registration

If you wish to register at the surgery you MUST live within the practice area (see map opposite). The only exception is new babies born to existing patients.  Due to the increasing numbers at the surgery we are unable to take any new patients who live outside our area, even if they are moving into existing patient households.

PLEASE NOTE KILNWOOD VALE IS OUT OF OUR AREA.

To register you need to provide:

  • Proof of your identity, e.g. passport or driving licence for online services
  • Proof of your new address to establish you live in our practice area e.g. bank statement, benefits form, rental agreement, etc.

Unfortunately, we will not be able to register you for online services without proof of ID.  

You will then need to complete a GMS1 form which is available from the reception and will also be given a new patient questionnaire to complete. If you are on any regular medication you will need to attend for a new patient health check. This check is also available to all new patients.

You will be given a choice of GP whom you will be registered.  Although you can see other GPs within the surgery, but for continuity of care we request you see with your registered GP for all routine appointments. As each GP specialises in various medical fields you may from time to time be referred to them for a particular medical complaint.
