CAR PARK - The Car Park resurfacing has now been completed and the car park is now open for use - we apologise that the white lines have yet to be added and the disabled spaces marked, however there is some other work to the car park which needs completing prior to this being done which we anticipate being completed in the next couple of months.

Our process for registering new patients:

Registration is by appointment only.

We would firstly ask you to call the surgery or pop in and confirm that you are living within our catchment area.

We can then provide you with a New Patient Information PackPatient Leaflet and GMS1 form or this can all be printed from our website.

At this stage we will then book an appointment for you to come to the surgery and complete the registration process.

This entails a 10 minute appointment per person registering with our Administration staff, all your documents will be copied and checked and we will enter your details on to the computer system.

We ask that you come to your registration appointment with all the correct documents and ensure that the Adult Health Questionnaire and GMS1 form have been fully completed.

If you arrive for your appointment without the correct documents you will not be registered and a new appointment will be booked for you to return with the required documents.

Children do not need to be present for the appointment as all documents will be completed by a parent/guardian on their behalf, however, you will need to bring their completed Under 16 Health Questionnaireform with you and their Red Book.

Adults must come in and register themselves.

We do not register any children at the practice without a parent/guardian being registered here.

We offer registration appointments on the following days/times

Monday/Wednesday/Friday 12 – 2pm

Tuesday/Thursday 11- 1pm

2 Saturdays per month (will vary) 9.30 -11am


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
