CAR PARK CLOSURE

The car park is closed today (12th Dec) due to the weather conditions - there is a pathway clear from the road to the front door please use this.

Christmas 2017 closure and New Year 2018 closure times.

We will be closed from 11.30am on Saturday 23rd December 2017 until 8am Wednesday 27th December 2017. We will also be closed from 11.30am on Saturday 30th December 2017 until 8am on Tuesday 2nd January 2018. If you require medical help during that time please ring 111 for non life threatening emergencies or 999 for life threatening emergencies. NHS Health Checks - guidelines for these checks have recently changed, we will be reviewing our processes, in the meantime we will not be booking any Health Checks - further information to follow.

Gluten Free Food Prescriptions

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require either a passport or birth certificate.

Proof of Identity requirements

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients on repeat medications are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
