New Patient Registration

If you live within our practice catchment area (map opposite) and wish to register with our practice you will first need to have a registration interview with the doctor.

Registration forms need to be completed before appointment with the doctor.  Proof of address, Passport/ID card for permanent stay in the UK or a valid visa of 6 months or over will be needed.  Parents with children 5 years and younger, need to bring the red book or any baby immunisation certificate.

Once registration has been accepted the receptionist will book an appointment with the Practice Nurse for a health check which should be done within 6 months of acceptance.

On registering you may wish to give preference to receiving care from a particular doctor.  However, please be aware that it may not always be possible to see the doctor of choice.
