New Patient Registration

To register with the practice you must live within the practice boundary (see map on the right).  You will be registered with the practice but we will allocate you to one particular doctor who will supervise your care.  Patients are able to express a preference to see a particular doctor and we will endeavour to comply, although this may not always be possible.  If you move out of our practice area you will need to register with another practice.

If you are taking regular medication you may need to be seen before we can issue a prescription. We do not prescribe for addiction and patients requiring this specialist service will be referred.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice. Please complete both documents.
