Confidentiality and Your Records
The practice operates a strict confidentiality policy. Information about patients will not be disclosed to any unauthorised person or organisation without the patient’s consent.
We have a duty to:
- Maintain full and accurate records of the care we provide to you
- Keep records about you confidential, secure and accurate
- Provide information in a format that is accessible to you
We will not share information that identifies you for any reason, unless:
- You ask us to do so
- We ask you and you give us specific permission
- We have to do so by law
- We have special permission for health or research purposes
- We have special permission because the interests of the public are thought to be of greater importance than your confidentiality
We may share information with the following main parner organisations:
- Primary Care Organisations
- NHS Trusts (hospitals or primary care trusts)
- General Practitioners (GPs)
- Special Health Authorities
- Ambulance Services
We may also share your information, with your consent and subject to strict sharing protocols about how it will be used, with;
- Social Services
- Educational Services
- Local Authorities
- Voluntary sectory providers
- The private sector
Summary Care Record
Your Summary Care Record (SCR) is a copy of key information held in your GP record. It provides authorised healthcare staff with faster, secure access to essential information about you - when you need unplanned care or when your GP practice is closed.
Summary Care Records will assist healthcare staff who may be treating you now and in the future. This can improve your experience of care and also the safety and quality of that care. Your SCR already contains important information about any medicines you are taking, any allergies you suffer from and any bad reactions to medicines that you have previously experienced. You may want other details about your care to be added to your SCR - this is called additional information and could include for example, your long term health conditions, immunisations, your health care preferences.
If you are happy for your information to be used in this way you do not have to do anything. If you wish to opt-out, you will need to contact the practice to let us know.

Care.data is a programme of work led by NHS England and the Health and Social Care Information Centre which will bring together securely, health and social care information from different settings in order to see what’s working really well in the NHS – and what can be done better. Using data in this way is known as data sharing for purposes beyond direct care, and the information will only be shared if it will benefit patient care.
Collecting and connecting information nationally will help to:
- better understand diseases and develop drugs and treatments that can change lives
- understand patterns and trends in public health and disease to ensure better quality care is available to everyone
- plan services that make the best of limited NHS budgets for the health and wellbeing of everyone
- monitor the safety of drugs and treatments
- compare the quality of care provided in different areas of the country
GP-led clinical commissioning groups in four parts of the country, known as pathfinder areas, are helping to develop the care.data programme. The programme will be rolled out in participating GP practices in the CCG areas of Blackburn with Darwen, Somerset, West Hampshire and Leeds North, West and South and East. Pathfinder feedback will be used to refine materials for the national roll out.
