Holbrook Surgery is fully computerised and medical records are held on computer. The surgery is registered under the Data Protection Act and complies with all current legislation and good practice guidelines.

This covers data shared with national screening programmes that reduce the risk of cancer and alike. You can opt out of these by writing into the surgery.

Subject Access Requests (SARs)for Medical Records
As a patient you can request to see your medical records. Such requests should be made through the practice by completing the following form.

There is not charge for this information unless it is deemed to be excessive.
If you are requesting access for a dependant or child over the age of 15 years we may need their consent under the Mental Capacity Act or Gillick competency depending on the circumstances.

Insurance, Solicitors and other third party requests (Access to Medical Records)
If you approach a third party they may request access to your medical records. They should ask you to sign a consent form. No information will be released without this written consent unless we are legally obliged to do so.
