SELF-REFERRAL SERVICES
Patients can self-refer to a variety of departments within NHS Tayside. You may be advised by the practice to complete a self-referral form. Below, you will find self-referral forms that you can print off and send directly to the relevant department.
MIDWIFERY (MATERNITY) SERVICES
Patients who have had a positive pregnancy test should make contact with the central booking team to begin their maternity care. Patients can make contact in a variety of way:

A midwife will then contact you to arrange your initial appointment. Where possible, these appointments will be before 10 weeks of pregnancy.
