Trinity Care is the name of our local in-hours and out of hours triage service. The service runs from Trinity Medical Centre, based in central Wakefield and covers patients registered at Crofton and Sharlston Medical Practice, Warrengate Medical Centre, Maybush Medical Centre, Trinity Medical Centre and Eastmoor Surgery.
From May 2016, Crofton and Sharlston Medical Practice joined Trinity Care to support local services including our Accident and Emergency units and also to ensure out of hours services for our patients, remain as local as possible for as much of the time we are currently able to cover.
During the day anyone asking for a GP appointment, may be placed on the triage list to be called back by experienced triage nurses who will make an assessment of your symptoms and will determine the most appropriate care for you. This may be a same day appointment with a GP, an appointment with a mental health nurse, an assessment by a physiotherapist, or you may be asked to visit a local pharmacy. Sometimes the nurses may suggest that you take some over the counter remedies and call back if your symptoms worsen or do not improve.
From 6.30 - 8pm on weekdays and 9am - 3pm at weekends and on bank holidays if you need to be seen urgently you should still phone our practice telephone number 01924 862621, but your call will automatically be directed to Trinity Care during these hours. They will assess your symptoms and if necessary you will be able to see a GP at Trinity Medical Centre. For calls outside of these hours will remain the same - for clinical advice, home visits that cannot wait until the surgery reopens, please call 111. For urgent assistance, please call 999.
The rota for weekends is staffed by GPs from the above surgeries so you will be seeing either a GP from this surgery or another local GP depending on rata coverage at the time.
Trinity Care is open 365 days a year including Christmas, Easter and New Year.
