***You can now request your sick note by calling us, completing an e-Consult online form or completing a form you can obtain from reception desk***

***Please note our new phone system now records all incoming and outgoing phone calls.***

***We are closed on All BANK HOLIDAYS in England and we also have some monthly half day closures due to staff training ***
