Changes to Contact Details

It is important that you notify us as soon as possible if there is a change to your address or contact details, even if you are not currently receiving treatment from the Surgery.

If you believe you may now be out of our boundary area, please double check under the New Patients tab where you can input your postcode.

If you are no longer within our practice boundary, please register immediately with a new GP, preferably before you have a need to see them, so the registration process can be completed in full and there is no delay in accessing a GP.

Remember to also inform the hospital of any changes if you are currently waiting for an outpatient appointment to be sent.
