COVID Status Certification

Please do not contact your GP Practice to ask for a Covid vaccination passport or status certificate. If you are travelling within the next 21 days please follow the link below, you will need your username (which was on your blue invite letter) for online access. If however you don't have your username please call the following number 0808 196 8565.

GPs and their teams cannot issue vaccination exemption certificates. Further information on the vaccination exemption certificates scheme, including information on safe vaccination and routes to obtain an exemption can be found on the Scottish Government and NHS Inform websites.

Missing Appointment??

If you believe you should have received an invitation to your COVID-19 vaccination appointment by now, or if you have lost your invitation letter, then please contact NHS Scotland directly on 0800 028 2816, in addition, any adult in Scotland can self-register for a Covid vaccine appointment. This includes people who have just moved to Scotland so don't need to wait until registered with a GP.
