Your right to a chaperone during your examination
Crofton chaperone policy - advice
Takes place most weeks, unless there is a TARGET afternoon that week. We will normally phone you a few weeks before to confirm arrangements. Often we will write you a letter with the appointment time. This appointment needs to be confirmed. If you do not confirm this appointment, we will not be expecting you.
Conditions appropriate for Minor Surgery are skin tags, most skin lesions not suspected of being cancerous, cautery of noses, removal of sebaceous cysts, injections to joints and tendons. Contact person for this service is our respiratory lead nurse or administration, who can be contacted via our reception. (But preferably NOT before 11am)
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

One of the changes within this district, introduced was that from 1st September 2013 all childhood vaccination and immunisations, are now administered by our practice nurse team and no longer by the attached health visiting team.
If you have a child requiring immunisation, a member of our admin team will invite your child to attend the surgery for vaccination. If this appointment is not convenient, please telephone Claire Midgley on 01924 866918 to rearrange the appointment.

As part of our working with other local GP Practices within our Network area, Crofton and Sharlston Medical Practice reviewed how it offered services to our young people registered at the practice and welcomed the opportunity to work with the local Youth Association Group - Growing Yorkshire's Future.

We are pleased to announce following an inspection and assessment we have received an award from the Youth Association Group for becoming a young people friendly practice.
Over the last 12 months or so, we have been working closely with other practices, the CCG (clinical commissioning group) and other services offering support to our young people. This section is to share with you things we have been developing or offering to this group.

The practice received a visit by the Young Inspectors who spoke to staff, management and a GP and then performed an inspection of the building. They also asked us to take part in a local survey asking young people what they thought of our services. The report produced and feedback was very positive and the practice is currently reviewing services offered.
The practice is pleased to announce that we have been awarded a plaque for both location sites, confirming we have met the standards they were looking for.
